When admission is planned
- Ask the ward about your admission date, where to go, required documents and personal belongings. Discuss any need for a companion in advance.
- Tell the team about your medicines and allergies. Consult the care team before taking your own medicines.
Review together before leaving
- Ask for a clear discharge summary, medicine instructions and home-care guidance. Ask again about anything you do not understand.
- Find out when and where follow-up will take place and how to track pending results.
